Description

The North Carolina Department of Health and Human Services (“NCDHHS” or “Department”), Division of Health Benefits (“DHB”), through Money Follow the Person Demonstration Project (“MFP”), seeks to procure an experienced and qualified partner to serve as MFP Transition Coordinator for the services defined herein. The selected Offeror shall be responsible for administering the Transition Coordination services to (i) provide statewide Transition Coordination services assisting eligible older adults, and adults with physical disabilities transition from qualified long-term care facilities into community-based living arrangements, and (ii) engage in outreach and education efforts with potential MFP Participants, family members, individuals and organizations that connect to the transition process, and stakeholders throughout the state that support an individual’s transition to a home and community-based setting.
